Name Changes

Name changes in existing passports are no longer permitted. New passports must be issued to reflect a name change. You must send a passport renewal application and your passport to the U.S. Embassy in Stockholm for processing. The Embassy address is listed at the bottom of this page under the heading, “Where to send the Application.”

Passport issued less than one year ago:

If your passport was issued less than one year ago, you apply for a name change without any charge. Please follow the steps below:

STEP 1: Passport with old name

  • Your U.S. passport issued in your old name (issued less than one year ago).

STEP 2: Name change documentation

  • Proof of your name change in Swedish, i.e. a document from the Swedish tax authorities (Skatteverket) in your name with a komplettering for the date and type of name change stating both old and new name, or
  • a U.S. court order stating both old and new name, or
  • an official U.S. marriage certificate in original.

STEP 3: Passport Application

  • Application Form DS-5504 (PDF – 99KB). Please disregard the instructions listed on pages 1-2 since they apply to passport processing in the U.S.

STEP 5: Send your Application

Please send your passport, the application form, name change document if applicable, and shipment label to the following address:

Since passports are such valuable documents, we recommend that you use registered mail (rekommenderad post) when sending a passport application to the U.S. Embassy.

Passport issued more than one year ago

If your passport was issued more than one year ago, you must apply for a new passport using the appropriate renewal passport application and pay the passport fee. You must submit proof of your name change, i.e. a document from the Swedish tax authorities (Skatteverket) stating both old and new name, or an official U.S. court order, or a U.S. marriage certificate in original.

Please note that you can apply for a new passport in your new name by mail if you are eligible for mail-in renewal.

Data Correction

If you notice an error in your passport due to a mistake committed by our office, we will issue you a corrected passport at no cost within one year of issue.  In order to correct your passport, please follow the steps below:

STEP 1: Passport

  • The passport containing the error.

STEP 2: Application Form 

  • A properly completed Application for a U.S. Passport: Name Change and Data Correction Form DS-5504 (PDF 74 KB).  If the applicant is under 16 years of age, one parent must sign the DS-5504 on his or her behalf.

STEP 3: Photographs

STEP 4: Evidence showing correct data

  • Acceptable citizenship and/or identification evidence showing the correct data.

STEP 5: Postage

STEP 6: Send your Application

Processing Time

Upon receiving a complete application, the processing time for a passport is 3-4 weeks.
